According to Guilford County Social Services Director Sharon Barlow, the “One Big Beautiful Bill,” which is now the One Big Beautiful Law, is projected to “significantly” impact Medicaid in Guilford County and North Carolina in ways that are very worrisome to local and state leaders.
That impact, Barlow told the Rhino Times, will come primarily through cuts to federal funding and the implementation of work requirements for able-bodied adults.
The changes are expected to lead to a reduction in Medicaid coverage for many North Carolinians, and could, she said, “destabilize” the state’s healthcare system – particularly when it comes to rural hospitals.
Advocates of the bill argue that the thrust of the effort is to see that all those who can work do work – or provide community service – in order to be eligible for the benefits. Opponents, on the other hand, argue that it will provide so much red tape that people who are legitimately entitled to benefits will be unable to receive them due to an inability to navigate the more complicated bureaucratic system.
There’s also the concern that some rural hospitals across the state and the country will close due to a lack of Medicare business and funding.
Chairman of the Guilford County Board of Commissioners Skip Alston told the Rhino Times that he is concerned the bill will increase costs that have to be paid by county government since it may require additional county staff to conduct more frequent eligibility checks, as well as to fill out other forms necessary to implement the new restrictions. On the other hand, one county social services employee who has studied the bill said it will likely cause so many county residents to lose Medicaid services that the county may actually end up needing fewer social services workers in the future due to a major decline in the number of people receiving Medicaid.
One person concerned about the implications of the bill for the health of county residents is Barlow, who leads the social services division for Guilford County and who has spent the last several years overseeing Medicaid expansion for Guilford County government.
In responses to questions from the Rhino Times, Barlow listed her concerns as follows:
“1. Reduced Federal Funding: The OBBBA [the big bill] is expected to cut federal Medicaid funding, which could force North Carolina to reduce or eliminate optional Medicaid benefits, including home- and community-based care for people with disabilities. This could negatively impact access to care for vulnerable populations, including those with disabilities and those relying on home-based care.”
Barlow said the work requirements in the new legislation will knock a lot of people off Medicaid. While many conservatives say that’s just fine, others argue that the health system will see increased cost because the health care provided to the uninsured will ultimately be borne by taxpayers. The big bill, Barlow points out, mandates work requirements for most able-bodied adults on Medicaid, requiring them to prove they’re working, attending school or volunteering for at least 80 hours a month.
“2. State officials estimate that these requirements could lead to the loss of coverage for over 255,000 North Carolinians,” Barlow added, “with some potentially losing coverage due to red tape and paperwork rather than unemployment.”
Like Alston, Barlow is very concerned about the cost to Guilford County, which is already in dire financial straits due to more than $3 billion in school bond debt and a wild spending spree on anything and everything that’s taken place over the last five years.
“Implementing these requirements will require a costly and complex system to verify beneficiaries’ employment status adding additional work to an already overburdened county system,” Barlow told the Rhino Times.
Here are some of her other concerns:
‘3. Impact on Hospitals and Rural Communities: Cuts to Medicaid funding could disproportionately affect rural hospitals, which often rely on Medicaid revenues to stay afloat.
The loss of Medicaid funding could also reduce access to care for all North Carolinians, regardless of their insurance status.
- Eligibility Changes: The OBBBA also includes provisions that could limit Medicaid eligibility for certain non-citizens.
This could result in a decrease in Medicaid coverage for some lawfully present immigrants, including refugees, asylees, and victims of trafficking.
- Potential for Medicaid Unraveling: The bill’s provisions, including work requirements and funding cuts, have the potential to significantly destabilize North Carolina’s Medicaid expansion and could lead to an unraveling of the program.”
In summary, she said, the bill’s impact on Medicaid in North Carolina is expected to be “far-reaching, with potential consequences for access to care, the financial stability of hospitals and healthcare providers, and the overall well-being of vulnerable populations reliant on the program.”
The bill will also affect the SNAP program in Guilford County, Barlow told the Rhino Times. That stands for Supplemental Nutrition Assistance Program – a program that used to be known widely as food stamps. In Guilford County, 41,326 households were recipients of Snap funding as of June 2025..
Barlow wrote: “The ‘One Big Beautiful Bill Act,’ now law, will significantly impact SNAP in North Carolina, likely leading to increased food insecurity and financial strain on families. Key changes include cost-shifting to the state, stricter work requirements, and potential benefit cuts. More than a million North Carolinians are food insecure, including 1 in 6 children. Four out of five families participating in SNAP in NC have either a child, a senior or an adult with a disability. Between 2019-2023, an average of 80 percent of SNAP households in NC included someone who is working. More than 46,000 NC Veterans benefit from SNAP.”
Barlow said that cuts to SNAP benefits – combined with rising food costs – are “likely to exacerbate food insecurity and poverty” across the state.

The issue at hand that conservatives ignore is the growing separation of wealth in this country where too many people who work require public assistance to have a minimum quality of life. For more and more American’s the cost of advanced education is out of reach, cost of owning a home is out of reach and any sense of personal savings for retirement is becoming more and more challenging when compared to my generation (am 55years old).
But don’t worry, we have fewer immigrants of color in our country.

You have to keep in mind that in the last four years, the current Democratic board increased the county’s budget FOUR TIMES MORE than the previous Republican Board did in its EIGHT YEARS of being in charge. The current board also oversaw the largest tax hike in county history by not adjusting the rate after the 2022 revaluation and looks now to do the same thing next year after the current real kicks in. The commissioners are all very nice but the Rhino Times has been covering the county for nearly four decades and never seen spending like in the recent half decade. PS, As for the wasteful spending at the federal level by the president, the article was not about that, but, since you mentioned it, the millions spent at the Florida swamp prison, the refurbishing of Alcatraz, and many other things are absurd uses of taxpayer money, and don’t get me started on the billion in pointless spending that taxpayers are paying for plane that will be used for maybe a year before being handed over to the president. I think the ballroom is being funded privately. The Rhino Times is also not pleased with the fact that Congress is exploding the national debt to a point where it is not clear we will be able to recover from.

Yes. I am stuck at the moment. Have a job I cannot get to because of prices. No food in the fridge and having all that because I need a roof over my head. I have been wanting to ask for help but feel it’s useless when people less fortunate can’t even get help. It takes money to make money. What have we come to? My disabled husband is working as much as he can and his body will not allow enough for him to get us by, however he’s expected to support us on minimum hours, the most he can handle with his health and has been denied disability 3 times. We cannot even waste the gas to go apply for help to survive so it’s like what is the point in trying. We need a better system. Especially for those of us that are trying our best we can.
Sorry for your struggles. Do seek aid. There are still a few in government who still actually care about the less fortunate.

Even if you work, many jobs don’t offer benefits to part-time (under 30 hours) and pay such low wagers that many in working class need SNAP even though the work. (almost 13% of all working Americans need federal aid to feed their families) Need to stop thinking the problem is laziness. It isn’t as the vast majority of people want to work and self-sustain but have complex barriers where they need a safety net to help them survive. Disabled family member, underpaying jobs, lack of reliable transportation, abusive spouse, living in a rural community with few steady job opportunities, etc… etc… etc…

GuilfordCounty Budget overruns aren’t the Federal Government’s problem and have nothing to do with Medicaid cuts. Medicaid is a state program with paperwork done by each County, not funded by the County. But it’s complicated because they decided to sell everyone on Medicaid Expansion. (Research it for yourself, Chris.)

You are missing the idea that costs increase when ER becomes your primary care. One, due to lack of preventive care, health issues progress to point where they are more expensive to address versus prevent (statin meds versus hearth surgery for example). Second, ER care is not effective at treating acute medical issues so the patient has to revisit again and again and again versus having a structure treatment plan they would have under Medicaid.

Wait, you think SNAP covers all costs of feeding children? The average benefit is $332 which for a family at best covers cost of feeding a family for a week if you cut corners. Also note that Trump cutting the DOE has also tried to cut the federal funds that support free breakfast and lunch programs. Only after our Governor pushed back did he restore SOME of the funding.

Most care is provided through HCBS programs. In NC, we have PCS, CAP/DA. Just contact DSS (if trump hasn’t cut the staff yet) and they can help you find a provider. No it isn’t always easy but the services exist.

This is the big lie behind the Big B***ugly Bill. This is not a new trick by conservatives. The work requirement isn’t new per se. The documentation and process for proving your qualification for these programs has been ramped up greatly in the name of stopping fraud that really doesn’t exists. Too many corporations like Walmart, chain restaurants, grocers and companies that hire part time and gig workers instead of full time employees have created a form of subsidized capitalism that forces local governments and taxpayers to supply their employees with SNAP and Medicaid to survive. Billion dollar corporations, especially, should not pay so little and hire workers for so few hours weekly that taxpayers must pick up the tab to ensure their children have enough to eat and can see a doctor when they’re sick. Corporate welfare is a huge issue and a primary cause of income inequality. Even many full time workers can no longer afford rent and food. Every year the gap widens and people who work hard at essential jobs are being priced out of rental markets near their work. With unaffordable housing and legal immigrants being caught up and deported along with those here illegally, I worry about farmers who need employees to plant and harvest food, the workers who look after our elderly and children and all those who work at jobs that don’t pay a living wage. While Walmart keeps declaring higher earnings and taxpayers have to feed their employees.
